  • Permanent role, full time hours M-F, work from home and office (Dandenong South)

About You
With proven experience in supporting individuals with complex disabilities and/or mental health needs, you are driven by helping people thrive in their communities through care plans that foster independence, resilience, and confidence. Your background includes case management roles where you’ve successfully supported clients to access services and resources in their community.
What you’ll bring:

  • A solid understanding of the NDIS Framework and Support Coordination.
  • Strong knowledge of community resources, particularly in areas like health, drug and alcohol support, legal aid, income assistance, employment, and recreation.
  • A passion for making a difference and the ability to adapt to changing environments.
  • Excellent written and verbal communication skills.
  • Emotional resilience, patience, and flexibility to meet challenges head-on.

We’re seeking someone who takes initiative, asks questions, and collaborates effectively in a work-from-home environment. You’ll be self-motivated, eager to learn, and able to work autonomously while remaining focused on achieving outcomes for your clients.

About the Role
As a Support Coordinator, you will manage a portfolio of clients, working closely with them to implement their NDIS plans. Your role will focus on connecting clients to appropriate supports, building their capacity, and fostering resilience. Our clients often have complex needs, including experiences of trauma, mental illness, or disability. Your goal will be to empower clients to achieve greater independence, development, and confidence by designing tailored support plans and collaborating with relevant services.
Key Responsibilities:

  • Conduct comprehensive needs assessments aligned with clients’ NDIS plans and personal goals.
  • Develop clear, actionable support plans to guide progress and outcomes.
  • Facilitate connections with service providers to ensure clients receive the right supports.
  • Build clients’ capacity, knowledge and confidence to achieve their goals independently.
  • Provide support and guidance to clients’ families and carers.
  • Collaborate with and maintain positive, long-term relationships local service providers, community groups, and stakeholders.
  • Maintain accurate case notes and reports as required by ermha365 and the NDIA.

It is a requirement of this position to meet all compliance requirements concerning NDIS Work Screening, Working With Children's Check, work rights and possess a current & valid driver’s licence.
