Job Description

  • Provide assertive outreach support to people with complex needs in the community
  • SCHADS Level 4 - $88,000-94,000 p.a. plus super & salary packaging options
  • Full Time Maximum Term until 30/06/2026, based in Northcote

About You
Are you motivated to improve the lives of those living with complex needs? Can you support others positively to achieve desired outcomes? Are you flexible and adaptable with a strong understanding of the traps people find themselves in when they become disassociated with support networks? Additional attributes we are looking for include:

  • Previous experience in assertive outreach, youth work, case management, mental health, justice or AOD
  • The ability to effectively and quickly build rapport with people and think outside-the-box to achieve positive outcomes
  • Flexibility, patience, confidence, initiative, street-smarts and emotional resilience
  • The ability to contribute positively within a team and work autonomously to manage your case load
  • Demonstrated capacity to work flexibly, possess the ability to manage competing demands, and able to effectively set boundaries
  • Qualifications in psychology, mental health, social work or other relevant fields

About the role
The Assertive Outreach Support program (AOS) is focused on delivering targeted and time-limited response to a small number of clients with complex functional needs, who experience significant service gaps and present an unacceptable risk of harm to others and/or self. The AOS program will deliver assertive outreach and case management services to clients referred by the Department of Families and Fairness (DFFH) across the North West Metropolitan Region of Melbourne. Within the program, the Support Facilitator AOS will support people to overcome barriers to accessing services and is provided flexibility in the place of residence or community, to build capacity for people with complex needs and improve their engagement with targeted interventions and services. Working under the leadership of the Practice Leader, the Support Facilitator will deliver practical support allowing clients to obtain their desired goals.
The Support Facilitator AOS role entails but not limited to:

  • Provide assertive outreach and psychosocial support to young people and adults who are experiencing service gaps 
  • Work with participants, clinical teams and other stakeholders (e.g. family) to access and engage with a range of supports
  • Develop, monitor and track recovery plans whilst encouraging engagement and reducing risk
  • Complete risk assessments and analysis 
  • Engage participants within the justice system to build and establish rapport and reduce risk 

It is a requirement of this position to meet all compliance requirements concerning NDIS Work Screening, Working With Children's Check, Working rights and possess a current & valid driver’s licence.
